Request a Copy of Your Medical Records
As part of our dedication to exceptional patient care, we have tried to make requesting a copy of your medical records as easy as possible. Here is all you need to do to request medical records:

Print and complete the Medical Records Request Form.
Click here for fees associated with obtaining your medical records. - Submit your completed form AND a copy of your photo ID to us in one of the following ways:
- Mail it to us at: Newman Memorial Hospital Attn: Medical Records 905 S Main St. Shattuck, OK 73858
- Fax it to us at +1 (580) 938-2608
- Deliver it to our Medical Records office, which you can find in the Basement of the hospital.
- Pick up your records. We’ll have your records available for you to pick up within seven days after we receive your request. Simply visit our Medical Records office any time after that. Our normal business hours are Monday through Friday from 8:00 AM to 5:00 PM. Please remember to bring a valid driver’s license or other photo ID. We cannot legally give out records without proper identification.
Fees associated with obtaining your medical records
Effective January 1, 2014
Per Oklahoma Statutes, effective on November 1, 2015, any requests for medical records from patients will incur the following charges:

- $0.50 cents for each page if copied on paper
- $0.30 cents per page if the entire record is requested and delivered electronically
- $5.00 per x-ray image, photograph image or pathology slide
- Postage and delivery fees if mailed.
- Requests from Attorneys, Insurance companies and those by subpoena shall be charged a base fee of $10.00 in addition to the per page charge, plus postage/delivery fee.
- Maximum fee allowed in $200.00 plus postage/delivery fee
